NCHSAA ATHLETIC PRE-PARTICIPATION FORMS MUST BE COMPLETELY FILLED OUT & SIGNED BY PARENTS & STUDENTS BEFORE A PHYSICAL CAN BE GIVEN (DO NOT substitute this physical form)...Physical Forms can be downloaded from the NHS School Website (under Athletics).
An Athletic Physical is REQUIRED BEFORE ANY athletic participation...including off-season & summer work-outs, team camps, passing leagues, etc.
Athletic Physicals are good for 390 days.
