Free Athletic Physicals will be given on Monday 6/4 at the Jacksonville High Gym at 3:00 pm.
In order to receive a physical, a student-athlete should have a completed physical form and one that has been signed by their parents and themselves in the appropriate places.
Please only use the approved NCHSAA/OCS Physical form which can be downloaded from the school website or picked up from the front office.
Don't pass up this great opportunity provided by your community!
REMEMBER...You must have an up-to-date physical on file with the Athletic Director in order to participate in any summer team workouts, passing leagues, summer leagues, etc.
